In order to serve your ministry more effectively and to maintain
an efficient and safe environment for our children, this form must
be completed and returned two weeks prior to date requested.

RSVP estimated number of children no later than the last service before your event.
Example: Home Improvements on Saturday RSVP the Wednesday prior.

***Nursery is provided for a maximum of three (3) hours.***
